چکیده

We read with interest the article by Gafton et al published in 2012. The authors illustrated the use of a dual-phase CT protocol for the enhancement characterization of parathyroid lesions and suggested the elimination of precontrast and delayed scan phases from 4D CT to reduce radiation exposure. In this letter, we describe the alternative approach of our institution to 4D CT. Our modified 4D CT protocol uses virtual noncontrast (VNC) images obtained from dual-energy CT-reconstructed venous-enhanced images to replace true precontrast scans. This process allows a complete examination of the contrast enhancement and washout characteristics of parathyroid adenomas on 4D CT, without reducing the number of scan phases. Here we present a 57-year-old female patient who was suspected of having primary hyperparathyroidism (PHPT) and was scanned by using our modified 4D CT protocol. The institutional review board approval was waived for this dose-saving study. The patient was referred for neck imaging using our modified protocol after negative technetium Tc99m-sestamibi scintigraphy findings, despite the presence of elevated total serum calcium and parathyroid hormone levels. A modified 4-phase CT scan was obtained, with a dual-energy scan acquired during the venous phase.

99mTc-MIBI SPECT/CT imaging contribution in the diagnosis of patients with hyperparathyroidism

Introduction: Hyperparathyroidism is presented with increased parathyroid hormone (PTH) secretion due to hyperfunctioning of one or more of the four parathyroid glands. Primary hyperparathyroidism (p-HPT) can be due to parathyroid adenoma, hyperplasia or carcinoma of the parathyroid gland. Secondary hyperparathyroidism (s-HPT) is usually a response to hypocalcaemia and consecut...

3-phase dual-energy CT scan as a feasible salvage imaging modality for the identification of non-localizing parathyroid adenomas: a prospective study

OBJECTIVES Accurate pre-operative imaging of parathyroid adenomas (PAs) is essential for successful minimally invasive surgery; however, rates of non-localizing PAs can be as high as 18 %. Multiphasic dual-energy CT (DECT) has the potential to increase accuracy of PA detection by enabling creation of paired material maps and spectral tissue characterization. This study prospectively evaluated t...

Single phase computed tomography is equivalent to dual phase method for localizing hyperfunctioning parathyroid glands in patients with primary hyperparathyroidism: a retrospective review

OBJECTIVE This study aims to compare the sensitivity of dual phase (non-contrast and arterial) versus single phase (arterial) CT for detection of hyper-functioning parathyroid glands in patients with primary hyperparathyroidism. METHODS The CT scans of thirty-two patients who have biochemical evidence of primary hyperparathyroidism, pathologically proven parathyroid adenomas, and pre-operativ...

بررسی 110 مورد هیپرپاراتیروئیدی اولیه در بیمارستان دکتر شریعتی و نتایج لوکالیزاسیون آدنوم توسط اسکن MIBI

Primary Hyperparathyroidism is a fairly common disease that is much more predominant in females. Treatment is surgery and includes removing the parathyroid adenoma or the hyperplastic parathyroid glands. Considering the difficulty of this operation procedure, localization test are utilized including ultrasound, MRI, CT scan, Thallium-Technesium subtraction scan, MIBI scan and a series of other ...

Absent radiotracer uptake in thyroid gland in parathyroid scintigraphy with 99mTc-MIBI: A case report

Localization of 99mTc- hexakis-2-methoxyisobutylisonitrile (99mTc-MIBI) by parathyroid adenomas is well known, and this warrants MIBI scan to evaluate suspected parathyroid adenoma in primary hyperparathyroidism. Typically, the radionuclide concentrates in both the thyroid gland and parathyroid adenoma in early images, and later on delayed images washes out slowly from the...
